Focus on Frailty

As we move closer to opening the new Midland Met hospital in Smethwick, there may be some changes to the way Sandwell & West Birmingham NHS Trust deliver its services. The Trust is holding a Focus on Frailty conversation on Tuesday, 6 June 2023 from 10 am to 12 pm, at Yemeni Community Association, Greets Green Access Centre, Tildasley Street, West Bromwich, B70 9SJ.

If you are a local person who lives with or cares for an older and/or frail loved one, they want to share their frailty plans with you and get your views. Staff within a residential or nursing home setting or staff that work in an acute, community or a primary care setting are also welcome.

They will be talking about:

what the current frailty (pathway) service looks like
what will be changing as we move to the new Midland Met hospital
the benefits of the new frailty (pathway) service
how we aim to keep frail patients in their own home and out of hospital
