Everyone Health has been commissioned by Sandwell Metropolitan Borough Council to deliver the new Tier 2 Adult Weight Management Service. The free 12-week programme will support residents living and working in Sandwell to lose weight, improve dietary intake and increase their physical activity levels.
The new service has started and referrals are being contacted and triaged within 48 hours of referral and have their Initial Assessment appointment within seven days of referral.
Structure of the programme. The Everyone Health programme is delivered by AFN Accredited Nutritionists and qualified Physical Activity Instructors and can be delivered virtually or face to face. Patients receive an initial appointment prior to starting the programme and will participate in weekly group sessions consisting of a 45-minute Nutrition talk and 45-minute physical activity sessions. Physical Activity sessions range from circuit-based sessions to seated exercises which is adapted to work at an intensity right for the group and the individual. Sessions include cardiovascular exercise to increase fitness and stamina and resistance exercise to improve strength and balance.
Participants will receive a copy Everyone Health’s Your Shape booklet to use alongside the programme, which supports behaviour change around healthy eating, weight management and physical activity to manage weight loss and how to maintain this long term. Service users will be signposted following completion of the programme to local community services to continue their journey and will be followed up at 6 months.
Who is eligible? Adults 18 years plus with a BMI of 30 or greater.
